I will pay by alternate method (mail, in person, etc.) but please enter my name. I understand that if I have not paid by 6:00pm the night prior to the tournament, I will not be eligible for ease-off draw, and will incur a $5.00 late entry fee.

By signing this form you release the Tournament Director, any and all persons associated with C.A.S.T. or associated with the operation or organization of this tournament from all liability or responsibility for any and all injuries, accidents, death, and loss or damage of physical property. Also by signing you are verifying that a minimum of $100,000 liability insurance is carried on the watercraft used in this tournament and is current and in good standing. Your electronic signature using a valid driver's license number or state issued I.D. will serve as your release signature.
